29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:    (a) On Top of the New York Traditions Bank Building - Two employees worked installing wooden roof sheathing on top of the new bank building.  These employees used personal fall arrest equipment for fall protection.  One of the employees had both ends of his 6-foot long shock absorbing lanyard fastened to the dorsal dee-ring on his harness.  This employee then tied off by knotting one end of a vertical lifeline rope to the middle of the 6-foot long shock absorbing lanyard.  The shock absorbing lanyard would not work reliably in the event of a fall in that, the shock absorber would not function because the dynamic forces created during a fall would not get exerted on the ends of the lanyard and absorbed by the device, exposing the employee to the hazard of excessive arresting forces, on or about July 31, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
