29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Derrymore Project Management:  An employee operated the TPA Model 1000 Shrink Wrapper machine and suffered a finger burn/laceration from contact with the unguarded hot knife seal bar.  The employer did not ensure that the hot knife seal bar was properly guarded to prevent employee contact with the point of operation, on or about 08/04/2014.    ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.334(a)(2)(ii): When there was a defect or evidence of damage that could expose an employee to injury, the defective or damaged item was not removed from service until the repairs and tests necessary to render the electric equipment safe had been made:  a) Derrymore Project Management:  A damaged flexible cord was used to power a fan and the insulated conductors were exposed at various locations on the cord, on or about 08/04/2014.  ABATED DURING INSPECTION NO ABATEMENT CERTIFICATION REQUIRED
