On or About July 31, 2014 A). Employees were utilizing a DeWalt brand 4" grinder to cut/trip electrical conduit pipe, which was not protected or fitted with the designed wheel guard in place.

Employees were utilizing flexible extension cords to energize a DeWalt brand portable grinder/cut off wheel that was not properly grounded because the ground pin of the cord had been broken off/removed.
