29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    a)  Shop Area - The Machine Casadei 14-inch Table Saw, Model Number KS 3200, Serial Number 90-67-237, was operated without a guard that covered the exposed portion of the rotating blade, exposing employees to severe cuts and potential amputations.  This condition was observed on or about July 31, 2014.

29 CFR 1910.213(h)(1):  The sides of the lower exposed portion of the blade shall be guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed:    a)    Shop Area - The Craftsman Table Saw, Model C48BC-102, did not have an adjustable guard covering the lower portion of the blade.  This condition was observed on or about July 31, 2014.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a)  Shop Area - The employer did not develop and implement a written lockout program to protect employees from the unexpected energization of equipment, including table saws, sanders, and other woodworking equipment, during servicing and maintenance of the equipment.
