29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   a) Roof of residence located at 12255 Ferncreek Drive, Alpharetta, Georgia. Employees, using a pneumatic nail gun to install roof shingles, without eye protection were exposed to injuries from flying debris.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:   a) Roof of residence located at 12255 Ferncreek Drive, Alpharetta, Georgia. Four employees working from unprotected steep slope, 10 in 12 pitch, roof were not wearing their personal fall arrest system in a correct manner. The attachment point of their body harnesses was not centered between the shoulders but was located at or near the lower back. After attaching a lanyard to the harness employees would then wrap the lanyard through the straps of the harness so that the attachment point to the lifeline would be in front of them.
