29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  On or about July 31, 2014, at a commercial steel erection project, the site Project Superintendent directing the work did not ensure that he and the workers engaged in welding activities were protected from unprotected fall hazards:  a) where there was a break in elevation of 19 inches from the second story landing to the upper landing surface; b) guard rails were not installed on the second story landing; c) mid-rails were not installed on the top upper landing area; and d) workers worked outside of the guardrails hanging on by one hand and welding.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:  On or about July 31, 2014, an employee on the rooftop of a steel erection construction project working with a grinder marking welding points and performing grinding work was not protected from fall hazards. The employee was exposed to a fall hazard of more than (6) feet to the ground below.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:   On or about July 31, 2014, at a steel erection construction project, an employee performing grinding work was not provided with a stairway or ladder where there was a break in elevation of approximately 4 feet from the awning to the roof top exposing the employee to fall hazards.
