29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    On or about July 31, 2014, an employee on the rooftop of a steel erection construction project performing welding work was not protected from fall hazards. The employee was exposed to a fall hazard of more than (6) feet to the ground below.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:    On or about July 31, 2014, at a steel erection construction project, employees performing welding work were not provided with a stairway or ladder where there was a break in elevation of approximately 4 feet from the awning to the roof top exposing employees to fall hazards.
