29 CFR 1910.132(f)(3)(iii): The employer did not retrain employees when inadequacies in the affected employee's use of PPE indicated the employee had not retained the requisite understanding.  On or about July 31, 2014, and at times prior thereto, an employee was observed transferring BAROID® 41(Crystalline silica, quartz) without the use of an approved respirator. Inadequacies in retaining the requisite understanding affected the employee's use of PPE and exposed him to health hazards such as but not limited to, acute and chronic nose, throat, and respiratory infections.
