29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access.        a) Lot #35 Star Light Lane, Milford, DE - employees were using Werner aluminum ladders that extended approximately 12-18 inches, and not at least three feet above the upper landing surface nor was it secured to prevent displacement and provided with hand holds, on July 31, 2014.
