29 CFR 1926.405(a)(2)(ii)(E): Lamps used in temporary wiring for general illumination were not protected from accidental contact or breakage:  LOCATION: 18th floor, 40 West 57th Street. TMG Project  On or about July 31, 2014.  a) Employees working throughout the project were exposed to electric shock hazards because the incandescent light bulbs used for temporary illumination were not protected from impact by materials or equipment.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:  Location: 40 West 57th Street, New York, NY, 18th Floor      On or about July 31, 2014  A). An employee was working on a portable, Baker type scaffold, the scaffold tipped and the employee fell to the floor. The scaffold casters were not locked to prevent the sudden rolling of the scaffold.
