On or about July 30, 2014, at 7590 Settles Walk, Suwanee, Georgia:  An employee performing siding work was exposed to a struck-by hazard from metal chips and nails while using a nail gun without wearing eye protection.

On or about July 30, 2014, at 7590 Settles Walk, Suwanee, Georgia:  An employee performing siding work was exposed to a fall hazard of approximately 10.4-feet when he used a 16-foot Werner aluminum extension ladder that had bent and damaged side rails that was not taken out of service.
