29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder(s) was/were not secured at its top to a rigid support, and/or a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder: Westside of roof, On or about August 01, 2014, employees were exposed to a fall hazard greater than 6 feet to the dirt ground below, while using a Louisville extension ladder, Model L-2121-28, which did not extend 3 feet above the landing, to access the roof.
