29 CFR 1926.451(b)(1): 29 CFR 1926.450(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports.  The employer does not ensure that each platform on all working levels of scaffolds was fully planked or decked between the front uprights and the guardrail supports.  This violation was observed on or about August 5, 2014 when employees were exposed to a fall hazard while doing masonry work form the second level of a scaffold without it being fully planked.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all working levels of scaffolds are fully planked or decked between the front uprights and the guardrail supports.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  The employer does not protect employees on scaffolds from falling when working more than 10 feet above lower levels.  This violation was observed on or about August 5, 2014, at the job site where employees was exposed to an approximate 15 foot fall hazard when working from a scaffold platform without being protected from falling.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when working on scaffolds 10 feet or more above lower levels.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  The employer does not ensure that employees have a ladder, stairway, or ramp to access a scaffold.  This violation occurred on or about August 5, 2014 when employees were exposed to a fall hazard while doing masonry work from the second and third level of a scaffold without adequate access.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees have proper access to the scaffold platform.
