29 CFR 1926.451(d)(2): Suspension scaffold outrigger beams, when used, were not restrained to prevent movement:  a)  Suspension scaffold provided for employee use was equipped with an outrigger beam that was not restrained to prevent movement.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(d)(4)(i): Suspension scaffold outrigger beams were not set and maintained with the web in a vertical position:  a)  The web of an outrigger beam on a suspension scaffold provided for employee use was not maintained in a vertical position.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
