29 CFR 1910.106(e)(6)(i):  Adequate precautions against the ignition of flammable vapors were not taken:      On or about July 28, 2014 a fuel tank was removed from a vehicle by cutting the mounting straps. No procedures were implemented to prevent the tank from falling and striking the concrete floor, which can create a spark capable of igniting fuel, that can spill or splash  from the tank.
