29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift; Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:     a)      On or about 08-06-14, employees were utilizing a JLG 40H aerial lift basket to access/egress a roof surface at fourteen (14) feet above compacted surface without wearing personal fall arrest systems.    b)      On or about 08-06-14, an employee who was working from a JLG 40H aerial lift basket at fourteen (14) feet above compacted surface was not utilizing a personal fall arrest system.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a)      On or about 08-06-14, employees were working on a low slope pitch roof with a roof edge fourteen (14) feet above compact surface on the south side and ten (10) feet above compacted surface on the north side without utilizing fall protection equipment.
