29 CFR 1910.22(b)(1):  Aisles and passageways were not kept clear and in good repairs with no obstruction(s) across or in aisles that could create a hazard:    On or about August 7, 2014, in the lawn mower storage area of maintenance shop, up to 16 bolts were protruding approximately 2 inches from the floor creating a trip and/or impalement hazard where workers pass through to move, store, or work on the lawnmowers or other equipment.

29 CFR 1910.132(d)(2):  The required workplace hazard assessment was not certified as being performed, identifying the person completing the assessment, and written certification of the date of the assessment:    On or about August 7, 2014, the employer did not certify in writing that the workplace assessment, designed to identify personal protective equipment (PPE) needed by employees to prevent injury, was performed.      NOTE:  The assessment shall be written, titled, dated and certified by signature.  The required documented training shall include the following:      1)      When PPE is necessary;  2)      What PPE is necessary;  3)      Limitations of the PPE;  4)      How to properly wear and use the PPE, and  5)      Inspection and care of the PPE
