29 CFR 1910.244(a)(2)(i): In the absence of a firm foundation, the base of the jack was not blocked:  a) Site - On or about 07/28/14, in the absence of a firm foundation, blocking was not used to prevent the bottle jack from sinking through the asphalt surface.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.244(a)(2)(iii): Loads were not cribbed, blocked or otherwise secured immediately after being raised with a jack:  a) Site - On or about 07/28/14, while the bus was raised by jacks so maintenance could be performed, cribbing was not used by workers to prevent the load from falling.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
