OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to combustible dust (butadiene/styrene copolymers) deflagration, explosions, and other fire hazards while working at/or near the indoor pulverizing system and associated dust collection equipment:    On or about October 6, 2015:   a) The indoor Powder King Mill (model PKA-120) lacked a means of fire protection.  b) The indoor first stage cyclonic air-material separator (cyclone) associated with the Powder King Mill lacked a means of        i) fire protection,        ii) explosion protection, and       iii) deflagration propagation prevention (isolation) for the upstream mill, the downstream second stage enclosureless air-material separator, and the cyclone's material discharge.  c) The indoor cyclone exhaust fan/blower associated with the Powder King Mill lacked a means of fire protection.  d) The indoor second stage enclosureless air-material separator (un-enclosed filter bags) associated with the Powder King Mill lacked a means of fire protection.
