29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: a) Maintenance, on or about 8/7/14: Oxygen and acetylene gas cylinders were stored together.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a) Fleet Shop, on or about 8/6/14: Extension cord used to charge electric pallet jacks was not provided with a continuous and effective path to ground in that the ground pin was missing from the plug. b) At the facility, on or about 8/6/14: Crown pallet jack, Model WP 2330-45, was not provided with a continuous and effective path to ground in that the ground pin was missing from the plug connector.
