OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crush-by hazards:    Location: 199 Andover Street, Georgetown, MA    On or about 6/12/14, the employer attempted to dismount from the seat of the Komatsu, model #PC50UU-2, serial #8768 excavator, when the employer's shirt caught and activated the controls to the bucket of the excavator, it caused the bucket to swing and caught and amputated the employee's right index finger who was attempting to rig the safety chain of a boat trailer to the bucket of the excavator.
