29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a)  At the worksite, employees conducting roofing activities were unprotected from falls of approximately 10.3 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    a)  At the south side of the house under construction, the extension ladder used for access to the roof did not extend at least 3 feet above the landing surface, exposing employees to falls of approximately 10.3 feet.    b)  At the west side of the house under construction, the length of the ladder did not allow it to extend at least three feet above the landing surface.  The ladder was not secured and a grasping device was not installed for employees to use.
