On or about May 22, 2014, Lot:  An employee was allowed to operate the company's Daewoo G25S propane powered industrial truck without having completed powered industrial truck training and evaluation.

29 CFR 1910.178(o)(1): Load(s) on powered industrial truck(s) were not stable or safely arranged:  A.  On or about May 22, 2014, Lot:  A powered industrial truck operator attempted to lift the company's Toyota battery powered pallet jack on the forks of the Daewoo G25S powered industrial truck.  The load was not secured to a pallet or otherwise stabilized.  The pallet jack fell from the elevated forks, injuring a nearby pedestrian.

On or about August 13, 2014, company-wide:  The employer had not developed and implemented a written hazard communication program which provided information to employees on labels and other forms of warning, safety data sheets, and employee information and training.  Employees use propane to power the Daewoo G25s powered industrial truck.
