29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  a)  680 Mulberry Dr. - On or about August 11, 2014, employees climbed a roof's leading edge, 20 feet above grade, to access a window opening located several feet below the upper surface being used for egress as opposed to using the available ladders installed along an adjacent roof field.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  680 Mulberry Dr. - On or about August 11, 2014, employees are working on a steeply pitched roof, approximately 24 feet above grade, without utilizing fall protection.  The Porras Brothers was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(d)(16)(iii), which was contained in OSHA inspection number 941882,  citation number 1 item number 1 and was affirmed as a final order on November 7, 2013, with respect to a workplace located at Birches at Newtown, Newtown, PA.
