29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked.    a. In the loading area, the employer did not ensure aisles or passageways were marked where there was pedestrian and forklift traffic in the same area.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  The Jegs Automotive, Inc company located in Delaware, OH:  a. On the 2014 OSHA 300 log, the employer did not ensure all the required information was included in column F for each entry. The entries did not include the substance/object that caused the injury.
