29 CFR 1910.147(c)(6)(i): The employer did not conduct periodic inspections of the energy control procedures at least annually to ensure that the procedure and the requirements of this standard are being followed:    a) On or about 08/12/2014, in the facility, the employer had not updated its LO/TO procedures since 2004 and had missing information on isolation devices and control valves on equipment such as, but not limited to, washers and dryers.
