29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury: (a). At Building #6 in the Lennar Townhome development of Wesley Chapel the employee pumping cement thru a 2 inch hose under pressure was not provided with eye or face protection to protect him from being struck-by splattering cement, on or about October 22, 2014.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a). At Building #6 in the Lennar Townhome development of Wesley Chapel where an employee walking the top of the concrete block wall of the two story building pumping cement into the wall was not protected from the 20-foot fall from the open side of the wall to the ground below, on or about October 22, 2014.
