29 CFR 1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question:    On or about August 5, 2014, erectors and scaffold inspectors were not trained in the proper assembly of the Sky Climber Rolling Roof Rig equipment, including but not limited to the correct placement of pins in the outrigger beams, exposing employees to the hazards of falls.
