29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  On or about August 13, 2014 and times prior thereto, a written respiratory protection program that included medical evaluation, training, cartridge change schedules and fit testing was not established and implemented for employees required to wear half face, tight fitting respirators.  Employees sprayed coatings that contained chemicals such as, but not limited to, barium sulfate at concentrations of 15 to 40 percent, and Kaolin at concentrations of 0.5 to 1.5 percent.  Employees were exposed to respiratory hazards.
