29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:   On or about August 14, 2014, employees were exposed to struck-by hazards when not wearing protective helmets, while other employees were working above them on the ceiling joist and top plates using pneumatic nail guns for installing the roof rafters.

29 CFR 1926.304(d): Each portable, power-driven circular saw withdrawn from the work did not automatically and instantly return the lower guard to the covering position of the saw:   On or about August 14, 2014, employee was exposed to the hazard of coming in contact with the saw blade, when using a Ridgid Model #R3203 serial #BBO69914087 hand held circular saw, where the lower blade guard had a wood shim wedged into the guard, keeping the guard in the open position, while employee was cutting the rafters.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about August 14, 2014, employees were exposed to falls greater than 6 feet in height to the concrete level below when installing the roof rafters to a two story residential home, while standing on ceiling joist and the top plate of the wall.
