29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:    a) Field Bleachers - Employees installing platforms on bleachers approximately 20 feet above the ground were not provided with fall protection; on or about 08/15/2014.    Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) Field Bleachers - Employees installing platforms on bleachers approximately 20 feet above the ground were not provided with fall protection. The employees did not recognize the hazard and means to correct the hazard, the employees were not trained; on or about 08/15/2014.    Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:    a) worksite - Employees were climbing the bleacher poles and ledgers to reach the working level approximately 5 feet above the ground; on or about 08/15/2014    Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
