29 CFR 1910.23(c)(1):  Open-sided floor(s) and/or platforms 4 feet or more above adjacent floor or ground level were not guarded by a standard railing(s) (or equivalent):   On or about August 13, 2014, employee of CC Forbes, LLC Rig #118 were exposed to fall hazards of approximately seven (7) feet while pulling tubing on a pump jack located in Odessa, Texas. a)  West side of rig floor. b)  South side of rig floor.

29 CFR 1910.23(d)(1)(iii):  Employer did not provide stairways less than 44 inches wide having both sides open, one stair railing on each side.  On or about August 13, 2014, employees of CC Forbes, LLC Rig 118, were exposed to fall hazards of approximately four (4) feet while pulling tubing from hole on a pump jack located in Odessa, Texas.  a)  Six (6) riser stairs from lower level leading to Rig #118 platform.  b)  Ten (10) riser stairs from lower level leading to Rig #118 rig floor.
