29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a)Guerrero, Raul at 5154 Andes Way, Denver, CO 80249: On and before August 16th, 2014, the exposing employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns. The condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed. When such extension was not available because of the ladder's length, a grasping device, such as a grab rail, was not provided to assist employees in mounting an dismounting the ladder: (a)Guerrero, Raul at 5154 Andes Way, Denver, CO 80249: On and before August 16th, 2014, the employer did not ensure where the extension of the ladder above the roof was not available, the exposing employer did not ensure that a grasping device was provided to assist employees with access and egress from the upper roof. The condition exposed employees to fall hazards of approximately 30 feet.
