29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection measure provided:         (a) New Concept Contractor at 3501 Yosemite Dr. Greeley, CO 80632: On and before August 12, 2014, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in re-roofing activities while working at the unprotected edges of a roof on a single-family home. Employees were not utilizing a fall protection system. Employees were exposed to fall hazards of greater than 6'.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:        (a) New Concept Contractor at 3501 Yosemite Dr. Greeley, CO 80632: On and before August 12, 2014, the exposing employer did not ensure portable ladders were extended at least three feet above the upper surface (roof) being accessed.  This condition exposed employees conducting roofing activities to falls from height in excess of six feet.
