29 CFR 1926.501(b)(1): Employees on a walking/working surface having an unprotected side or edge which is six feet or more above a lower level were not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system: a) Route 46, Mile Marker 2, Columbia, NJ 07832. The employer allowed his employees to work in close proximity to 42 inch diameter holes, ranging from 12 foot to 18 foot deep with no fall protection systems in place exposing the employees fall hazards. Condition occurred on or about 08/13/14.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: a) Route 46, Mile Marker 2, Columbia, NJ 07832. The employer allowed his employees to work at the bottom of 42 inch diameter holes, ranging from 12 foot to 18 foot deep when materials were not at least 2 feet back from edge with no retaining device was in place to prevent that material from falling into holes exposing the employees to struck by hazards. Condition occurred on or about 08/13/14.
