29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  On or about June 11, 2014, and at times prior thereto, employees did not recognize the hazards of being struck by falling objects in that they were performing post-tensioning operations directly under an unsecured storage area holding trusses.

29 CFR 1926.250(b)(1): Material inside building(s) under construction was stored within 6 feet of hoistway(s) or inside floor opening(s):  On or about June 11, 2014, and at times prior thereto, employees performing post-tensioning operations were working underneath a load of trusses that were stored against the second floor of a residential home under construction, exposing employees to the hazards of being struck by falling objects.
