29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):      (a) On or about August 19, 2014, and at times prior, Precision West Signs, LLC., did not initiate and maintain a program which provided for frequent and regular inspections of job sites, materials, and equipment by a competent person. Employees were exposed to contact with electrical hazards while installing commercial signs that require excavating and digging into the ground, employer does not inspect the work sites regularly to ensure that employees are not exposed to safety hazards.        Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.651(b)(2): Utility companies or owners were not contacted within established or customary local response times, advised of the proposed work, and asked to establish the location of the utility underground installations prior to the start of an excavation:    (a) On or about August 19, 2014, and at times prior, Precision West Signs, LLC., did not ensure that underground utilities were located and marked prior to the start of an excavation, employees were exposed to electrocution hazards while digging into the ground to install commercial real state signs.
