29 CFR 1926.756(a)(1): During the final placing of solid web structural members, loads were released from the hoisting line that were not secured with at least two bolts per connection, of the same size and strength as shown in the erection drawings, drawn up wrench-tight or the equivalent as specified by the project structural engineer of record, except as specified in paragraph (b) of this section:      a) Jobsite, 12620 Perry Highway, Wexford, PA 15090:  Observed on or about August 05, 2014, beam number 2108 (beam type W12X14, knife connection) was not secured with at least two bolts per connection.  The beam was attached to column 2053 with one bolt.  An employee was injured when this beam connection fell.  Employees working below the beam were exposed to struck-by and crushed-by hazards.    b) Jobsite, 12620 Perry Highway, Wexford, PA 15090:  Observed on or about August 05, 2014, beam number 2112 (beam type W21X44) located between C and D lines was not secured with at least two bolts per connection.  One end of the beam was connected with one bolt.  Employees working below the beam were exposed to struck-by and crushed-by hazards.    c) Jobsite, 12620 Perry Highway, Wexford, PA 15090:  Observed on or about August 05, 2014, beam number 2105 (beam type W18X35, knife connection) located south of 2112 was not secured with at least two bolts per connection.  One end of the beam (knife connection) was connected with only one bolt.  Employees working on or near the beam were exposed to struck-by and crushed-by hazards.    d) Jobsite, 12620 Perry Highway, Wexford, PA 15090:  Observed on or about August 05, 2014, beam number 2080 (beam type W16X26, knife connection) located between C and D lines was not secured with at least two bolts per connection.  One end of the beam (knife connection) was connected with one bolt.  Employees working below the beam were exposed to struck-by and crushed-by hazards.
