29 CFR 1910.178(n)(7)(iii): 1910.178(n)(7)(iii):  On all grades the load and load engaging means were not tilted back if applicable, and were not raised only as far as necessary to clear the road surface.   Jobsite:  While ascending a grade the operator of the truck traveled with the forks a height of approximately 14 feet above the ground exposing the employees to roll over and crushing hazards.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   Jobsite:  An employee was exposed to fall hazards when he was installing roofing while working from a pallet that was on the forks of rough terrain fork lift.     ENH Restoration LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 10917769, citation number 1, item number 4 and was affirmed as a final order on 8/12/2010, with respect to a workplace located at 2 Sherwood Lane Cheshire, CT  06410.     ENH Restoration LLC was previously cited for a violation of an equivalent occupational safety and health standard  29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 910699, citation number 1, item number 1 and was affirmed as a final order on 23 July 2013, with respect to a workplace located at163 Concord Street East Haven, CT.     On or about 15 August 2014 an employee was injured and transported to the hospital after sustaining injuries due to a fall.
