29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolding was not fully planked or decked: On the south side of the scaffold located above the roof there was only a single plank installed where it takes five to fully plank the scaffold. Employees were exposed to a fall hazard of approximately twelve (12) feet to the sloped roof.

29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees: Stacks of styrofoam were used to support scaffold planks in an effort to extend the reach of the employees. Employees were exposed to a fall hazard of approximately forth (40) feet from the scaffold to the ground.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: A railing was not installed in all locations where a brace and a railing were required to provide the required fall protection. Employees were exposed to fa fall hazard of approximately thirty (30) feet from the scaffold to the ground.
