29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    The employer did not establish a written respiratory protection program for employees who were required to wear respirators. Employees were using respirators to perform their work and the employer did not ensure the employees were properly trained and medically fit to use respirators.       Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903(c).

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.     Employees were driving forklifts and were not adequately trained exposing employees to crushing hazards.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

The employer did not ensure that the point of operation was guarded for the tail and head stock stretcher to prevent employees from having their hands and fingers in the zone of danger during operation.       B. The employer did not provide a method of guarding on the finish saw to prevent employees from having their hands and fingers in the point of operation during operating cycles.
