29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:     a) On or about 8/21/2014, Rear of the house:  Employees were performing roofing work on an approximately 9 on 12 pitch roof with no fall protection in use.   Employees were exposed to falls of up to 8.5 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) On or about 8/21/2014, Rear of the house:  Employees were accessing a roof using a step ladder that only extended six inches past the eave.   Employees were exposed to falls of up to 7.5 feet.
