29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:   (a) On or about August 19, 2014, and at times prior, for employees in kitchen area cleaning the ovens, floors, and dishwashing when using hazardous chemicals such as but not limited to Sysco Keystone oven and grill cleaner and Pure Bright germicidal ultra-bleach and were not required to wear available, protective eye equipment at the Clarkson Health Care facility located at 1015 Mountain View Road in Rapid City, South Dakota.

29 CFR 1910.1030(d)(4)(iii)(A)(2)(i): During use, containers for contaminated sharps were not easily accessible to personnel or located as close as was feasible to the immediate area where sharps were used or could be reasonably anticipated to be found:   (a) On or about August 19, 2014, and at times prior, for employees working in the soiled linen rooms without easily accessible containers for contaminated sharps at the Clarkson Health Care facility located at 1015 Mountain View Road in Rapid City, South Dakota.
