29 CFR 1926.503(a)(1): The employer did not provide a training program enabling recognition of and appropriate protective procedures for fall hazards for each employee who might be exposed to fall hazards:    (a)The L Corporation, dba Formula Roofing and Restoration, at 3596 Wabash Street, Denver, CO: On and before 8/20/14, The L Corporation did not provide a training program enabling recognition of and appropriate protective procedures for fall hazards for each employee who might be exposed to fall hazards.  Five employees were re-roofing a two story home.  The employees were working upon the roof of the second story.  The employees had not been provided fall protection training.  This condition exposed the employees to a fall hazard of approximately twenty feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure:    (a)The L Corporation, dba Formula Roofing and Restoration, at 3596 Wabash Street, Denver, CO: On 8/20/14, the employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, personal fall arrest systems. Five employees were re-roofing a two story home.  The employees were working upon the roof of the second story.  Employees were working without an appropriate fall protection system.  This condition exposed the employees to a fall hazard of approximately twenty feet.    The L Corporation, dba Formula Roofing and Restoration was previously cited for a violation of this Occupational Safety and Health Administration Standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection 191524, Citation 1, Item 1a and was issued on April 12, 2012, and affirmed as a final order on April 26, 2012, with respect to a workplace located at 1368 Mariposa Street, Denver, Colorado.
