Five employees were re-roofing a two story home.  The employees were working upon the roof of the second story.  Employees were working without an appropriate fall protection system.  This condition exposed the employees to a fall hazard of approximately twenty feet.    The L Corporation, dba Formula Roofing and Restoration was previously cited for a violation of this Occupational Safety and Health Administration Standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection 191524, Citation 1, Item 1a and was issued on April 12, 2012, and affirmed as a final order on April 26, 2012, with respect to a workplace located at 1368 Mariposa Street, Denver, Colorado.
