29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system.    Location -  34 Malden Street, Everett, MA;  On or about 8/20/2014, an employee repairing a fascia board from a ladder jack scaffold that was greater than 10 feet above a lower level was not protected from falling to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.      Location - 34 Malden Street, Everett, MA;  On or about 8/20/2014, an employee accessing a ladder jack scaffold platform was using a ladder that did not extend three feet above the scaffolds platform.
