29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident  a) Directory Residence - 12012 71 St. Street East  Parrish, Fl. 34219  A worker experienced a heart attack while leaving the attic crawl space to go to lunch and was transported via ambulance to Manatee Memorial Hospital. The worker died while in the hospital on 08/19/14 but the incident was not reported to OSHA until 08/21/14.
