29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about August 22, 2014, and at times prior thereto, at the job site located at 1656 Industrial Road in Malvern, Arkansas, employees stripping and replacing a shingled roof were not using any form of fall protection. These conditions exposed the employees to the hazard of falling approximately 18 feet to the ground below.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  On or about August 22, 2014, and at times prior thereto, at the job site located at 1656 Industrial Rd in Malvern, Arkansas, employees who had not received any training regarding fall hazards, were stripping and replacing a shingled roof, and were not using any form of fall protection. These conditions exposed the employees to the hazard of falling approximately 18 feet to the ground below.
