29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure:  (a)GTO Roofing at 3592 Willow Street, Denver, CO: On 8/20/14, the employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, personal fall arrest systems. Five employees were re-roofing a two story home.  The employees were working upon the roofs of the first story and second story.  Employees were working without an appropriate fall protection system.  This condition exposed the employees to a fall hazard of approximately twenty feet.

Five employees were re-roofing a two story home.  The employees were working upon the roofs of the first story and second story.   The employees had not been provided fall protection training.
