29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: a) Back Porch - Employees accessing and egressing from the roof were exposed to a nine (9) foot fall hazard from a ladder that was not extended at least three (3) feet above the upper landing surface, on or about August 28th, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems: a) Roof - Employees ripping off shingles on an eight (8) by twelve (12) pitch roof were exposed to a fall hazard of approximately thirty five (35) feet without the use of a guardrail systems with toe boards, safety net systems, or personal fall arrest systems, on or about August 22nd, 2014. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records. Wolf & Associates LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 949303, citation number 1, item number 1 and was affirmed as a final order on 12/30/2013, with respect to a workplace located at 1955 Warrensville Road, Mountoursville, PA.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
