29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: A) 78 Lafayette Street, Carteret, NJ 07008 - Tank 87 : Employees were exposed to fall hazard of approximately 8 to 9 feet from the working level to the ground due to missing planking on a tube and coupler scaffold during sandblasting operations. Condition occurred on or about August 18, 2014.

29 CFR 1926.452(w)(6)(i): Employee(s) were allowed to ride on scaffolds when the surface on which the scaffold was moved was not within 3 degrees of level, and/or not free of pits, holes, and obstructions: a) 78 Lafayette Street, Carteret, NJ 07008 - Tank 87: Employee was allowed to ride on a tube and coupler scaffold while it was being moved. The surface on which the scaffold was being rolled contained obstructions, including but not limited to weld lines and sand, exposing employee to fall hazards. Condition occurred on or about August 18, 2014.
